Service Description

If you need help deciphering your medical records, I can review to see if there may be errors or services you can appeal. This service is included if you engage me to work on your case.

Cancellation Policy

For Consultancy Agreement: To cancel your contract prior to the conclusion of your settlement, the $250 deposit will be forfeited. Pending rewards or reimbursement you receive as a result of my work will be billed at the 20% rate. Communication with your providers and insurance company will cease, and any project notes provided to you. It will be your responsibility to remove Jonna Jerome & My Patient Voice from any Release of Personal Information on file. All other services: Please cancel or reschedule your appointment 24 hours in advance.
