Consultancy Agreement

Service Description

This Agreement includes every service I offer except the 1-hour conference call. Rather than "a la carte" pricing, you pay only a $250 deposit for me to begin work, and a "success fee" of 20% of the savings I procure for you. If no savings are achieved, you will not pay the 20% fee. An invoice for the deposit is forwarded when you indicate you wish to engage my services, along with a Service Agreement. Work will commence when I receive your deposit and signed contract.

Cancellation Policy

For Consultancy Agreement: To cancel your contract prior to the conclusion of your settlement, the $250 deposit will be forfeited. Pending rewards or reimbursement you receive as a result of my work will be billed at the 20% rate. Communication with your providers and insurance company will cease, and any project notes provided to you. It will be your responsibility to remove Jonna Jerome & My Patient Voice from any Release of Personal Information on file. All other services: Please cancel or reschedule your appointment 24 hours in advance.
